JOINING CCPH
- You can join CCPH online here. This form accepts credit card
AND check payments. Please contact info@ccph.info if you have any difficulties with
the form.
How do I decide which membership category is right
for me?

Individual e-Membership
If you are interested or involved in any aspect of community-campus
partnerships and want to improve the health of communities, this
membership category will quickly connect you with essential contacts
and resources. Created to enable our members to take advantage of those
benefits most important to them, the Individual e-Membership includes
subscription to our e-newsletters, use of our online membership
directory, publication discounts, access to Member Interest Groups, and
customized emails tailored to your interests. For members outside North
America or those who may not be likely to attend CCPH conferences or
institutes, the e-Membership is often the best choice. The annual cost
of an Individual e-Membership is $90.
Individual Premium
Membership
This membership category was created for individuals who want to take
full advantage of all of CCPH membership benefits and services.
Benefits include all of those offered under the Individual
e-Membership, plus discounts on registrations for CCPH events. The
annual cost of an Individual Premium Membership is $110.
Individual Student
Membership
If you are currently enrolled full-time in a college or university and
involved in community-campus partnerships, the Individual Student
Membership is a great deal for you. Benefits of the Student Membership
level are the same as those for Individual Premium Membership. Students
must submit a copy of their student identification to verify their
status. The annual cost of an Individual Student Membership is $40.
Organizational
Membership
If your organization has multiple individuals involved in
community-campus partnerships, then the Organizational Membership is
for you! Designed to offer maximum value, this membership allows four
people affiliated with your organization or partnership to take
advantage of the membership benefits and services offered under the
Individual Premium Membership. In addition, organizational members
receive one hour of free phone consultation on a topic of their choice,
as well as substantial discounts on CCPH Consultancy Network services.
The annual cost of an Organizational Membership is $330. More than four
people can be listed for a fee of $90 per additional person.
How do I find out if my organization is already an
organizational member of CCPH?

There are two ways:
- For an immediate answer, use our online member directory.
Both guests and CCPH Members can search the directory, but members have
access to more search options and more complete information on current
CCPH Members.
- Access the directory at http://web.memberclicks.com/mc/prelogin.do?hidOrgID=ccph
- To use the directory as a Guest, there is no need to log
in. Just go directly to the left-hand link labeled "Directory". Choose
"Simple Search."
- Members may want to log in with their username and
password—doing so takes users directly to the Simple Search page.
- On the Simple Search page, enter the name of the
organization or institution you are searching for in the search term
box. If you don’t see a record for the organization and institution, do
a search on the city or state/province where the organization is based
and then browse through the results.
- The search results will show the name and location of
members, as well as their CCPH Membership Type.
Tip: If you perform more than one
search, be sure to click on the "Clear" button between searches on the
Advanced Search page, or clear out the search box on the Simple Search
page.

How do I find out who from my organization is
already a CCPH member?

Are members on the same CCPH Organizational
Membership required to have the same employer?

No. In fact, we encourage people working
together in community-campus partnerships to join CCPH together as
Organizational Members! When you join as an Organizational Member, our online form will provide a
space for you to provide contact information for the individuals associated with
the membership. This will ensure they all receive benefits.
Is it possible to join CCPH for several years at
once? I have some grant money that I'd like to use up!

Yes! To join as a multi-year member, paying either via check or credit card, please contact
our membership coordinator. We would be happy to help you with a multi-year membership transaction.
How can I be added to my organization’s
Organizational Membership?
|
Each Organizational Membership has a primary
contact person who determines who is affiliated with that CCPH
Organizational Membership. The best way to join an existing
Organizational Membership is to talk to the primary contact person. To
find out who the primary contact person is, you can contact the CCPH membership coordinator, or
find out from those whose names you may have seen in our online Member
Directory.
Other things to keep in mind:
- Organizational Memberships can have more than four
individuals affiliated with them, for an additional $90 per additional
member.
- Consider forming your own CCPH Organizational Membership if
you have colleagues who may also be interested, or who may already be
Individual Members.
- Individuals affiliated with a CCPH Organizational
Membership do not need to have the same employer.
Our institution requires a Vendor form/W-9/Federal
Taxpayer ID number. How do I get that information?

Our Federal Taxpayer ID number is
94-3285533.
If you need a completed W-9 form, please contact our membership coordinator.
What forms of payment do you accept?
|
We accept checks drawn on US funds and these credit cards: Visa, MasterCard, Discover, and American Express.
We
do not accept purchase orders or debit cards. If your institution
requires an invoice in order to process checks through its system,
please contact our membership coordinator.

members to actively participate in shaping our programs, projects, and
services. Members serve as advisors on program planning committees, as
participants, speakers and facilitators during our conferences &

USING MEMBER
BENEFITS
I don't know my username and password. How do I
obtain them?

Members
receive their username and password in the welcome email sent to all
new and renewing members. If you no longer have this information,
retrieving it is easy! Our online Member Directory stores this
information and will re-send it to you upon request:
- Visit the log in page: http://web.memberclicks.com/mc/prelogin.do?hidOrgID=ccph.
- Click on "Forgot your Password?"
- Enter the email address you gave CCPH when you joined or
last renewed
- Your username and password will both be sent to the email
address we have listed for you.
- If your username and password are not sent to you, or you
encounter difficulties, contact our membership
coordinator.
What are Member ID numbers?
|
Member Identification (Member ID) numbers
are unique numbers assigned to each person who joins CCPH
(including each person who is connected to an Organizational
Membership). Member ID numbers are sent in emails confirming membership
when you join or renew. Member ID numbers are not transferable—they
uniquely identify individuals, and they remain the same for members who
rejoin even if their membership has lapsed for several years.
CCPH uses Member ID numbers to make taking
advantage of membership services and benefits as easy as possible for
members. Member ID numbers help us quickly verify active memberships
and provide you with access to discounts on event registrations,
publications, member-only web resources and much more! If you are a
CCPH member, please keep your Member ID number handy for easy reference
when you are registering for a CCPH event, purchasing a publication,
etc.
Please note that Organizational Members will
need to use their Member ID numbers (and not the Organizational ID
number) when consuming CCPH Member benefits.
I don't know my Member ID. How do I find out?

I'm trying to access instructions on redeeming
CCPH Member publication discounts. How do I get the password?

New
and renewing CCPH members receive the member publications discount
password via email when their membership is confirmed. If you are a
CCPH member and need the password, e-mail info@ccph.info.

I have an announcement I would like to publicize
to CCPH Members. How do I get the word out?

There are three ways we can help you get
your message out.
- Post your announcement to an appropriate listerv: CCPH
moderates (and participates in) several listservs related to current
CCPH projects and topics of interest. Descriptions and subscription
information are available at http://depts.washington.edu/ccph/faq.html#ListservFAQ.
- Announcements that are appropriate for CCPH's newsletter
Partnership Matters can be included in an upcoming issue. Please
contact the editor for information on upcoming publishing deadlines and
details on how to submit your announcement.

If you have items to submit, please
follow these guidelines:
- Limit announcements and questions to not more than 100
words. As for articles and editorials, not more than 200 words.
- Limit articles and editorials to not more than 200
words.
- Provide the names and affiliations of any authors.
- Include photos if possible.
- Explain all abbreviations and unusual terms when first
used.
- Submit time-sensitive items at least one month in
advance.
- If you are a CCPH member, another way to send your message
is to use the Online
Membership Directory. You can send the message to all members, or
you can use the search options to limit the broadcast—to people in
certain states for instance. This is not a moderated list, so your
message goes out immediately, can be of practically any length and can

RENEWING YOUR
MEMBERSHIP

How do I change the names listed on my
Organizational Membership

To change the names listed on your CCPH Organizational Membership, please download, complete and return the Member_Replacement form.
If you would like to change the names listed on your membership while renewing your membership, simply update the names and contact information when prompted by the online form.
I am a CCPH member but have not received my username and password and I am not listed in the online directory. Why?
|
There are several possible reasons for this:
Your membership may have expired and you are not currently a CCPH
member. To check on the current status of your membership, please see How do I find out if my CCPH membership is current?
We may not have your current contact information. If you think this might be the case, you can update your contact information.
